Based on the facility walk through inspection at the main hospital campus at 16088 San Pedro, San Antonio, TX, with the hospital administrator and facility engineer, it was noted that two sets of cross-corridor smoke barrier doors lacked astragals as required by NFPA 101(2000) 18.3.7.8.

Based on observations during the facility walk through inspection with the facility engineer, at the main campus on 16088 San Pedro, San Antonio TX, , on 01/22/2015, it was noted that a small noursihment counter with a microwave oven and coffee maker, was located in an alcove of the main egress corridor. This area was not protected by a ceiling mounted smoke detector, connected to the building fire alarm system as required by NFPA 101 (2000) 18.3.6.1.

Based on observations during the facility walk through inspections, on 01/22/2015, with the facility engineer and hospital administrator, at three hospital campuses;
(1) the main hospital campus on 16088 San Pedro, San Antonio TX,
(2) the campus at 25615 North Highway 281, San Antonio TX
(3) the campus at 8230 North Loop 1604, SAn Antonio TX,
it was noted that there were no remote annunicator panels for the emergency generator located at the engineering/maintenance office/areas as required by NFPA 99 (1999) 3-4.1.1.15.

Based on observations during the facility walk through inspections with the hospital administrator and facility engineer, on 01/22/2015, at the main hsopital campus, it was noted that there was not a medical gas master alarm panel located at the facility engineers office.or wrok area as required by NFPA 99 (1999) 4-3.1.2.2 (b)2.

Based on observations during the facility walk through inspections on 01/22/2015 with the hospital administrator and facility engineer, at the main hospital campus at 16088 San Pedro, San Antonio TX, it was noted that duplex receptacles serving patient care areas, powered from the critical branch of the EES were not permanently marked to identify electrical panel and circuits supply ing power as per NFPA 99 (2000) 3-4.2.2.4. Identification markings were adhered with paper tape, easily removed.
