Interior Wall and Ceiling Finish

Based on observation and interview; the facility failed to ensure 1 of over 50 rooms were provided with a complete interior finish with a flame spread rating of Class A or Class B for a sprinklered facility. LSC 10.2.3.4 states products required to be tested in accordance with ASTM E 84, Standard Test Method for Surface Burning Characteristics of Building Materials or ANSI/UL 723, Standard for Test for Surface Burning Characteristics of Building Materials shall be grouped in the following classes in accordance with their flame spread and smoke development.
(a) Class A Interior Wall and Ceiling Finish. Flame spread 0-25; smoke development 0-450. Includes any material classified at 25 or less on the flame spread test scale and 450 or less on the smoke test scale. Any element thereof, when so tested, shall not continue to propagate fire.
(b) Class B Interior Wall and Ceiling Finish. Flame spread 26-75; smoke development 0-450. Includes any material classified at more than 25 but not more than 75 on the flame spread test scale and 450 or less on the smoke test scale.
(c) Class C Interior Wall and Ceiling Finish. Flame spread 76-200; smoke development 0-450. Includes any material classified at more than 75 but not more than 200 on the flame spread test scale and 450 or less on the smoke test scale.
This deficient practice could affect over 10 residents, staff and visitors in the vicinity of Room 209 on the second floor in the Residential Unit.

Findings include:

Based on observations with the Director of Risk Management and the Plant Operations Director during a tour of the facility from 9:55 a.m. to 11:20 a.m. on 10/24/19, the ceiling of the bathroom for Room 209 on the second floor in the Residential Unit consisted of plywood sheeting. In addition, three areas of the walls in Room 2019 had plywood sheeting affixed to the wall. Interior finish flame spread rating documentation was not affixed to the sheeting by the manufacturer. Based on interview at the time of the observations, the Plant Operations Director stated the plywood sheeting was affixed to ceiling and walls within the last two days to repair wall and ceiling damage caused by the resident, he did not treat the plywood sheeting with flame retardant material and agreed interior finish flame spread rating documentation for the plywood sheeting interior finish was not available for review.

Portable Space Heaters

Based on observation and interview, the facility failure to ensure 1 of 1 portable space heaters were not used in health care occupancies. This deficient practice could affect over 10 residents, staff and visitors in the vicinity of Classroom 1.

Findings include:

Based on observations with the Director of Risk Management and the Plant Operations Director during a tour of the facility from 9:55 a.m. to 11:20 a.m. on 10/24/19, an operable electric portable space heater was plugged into a power strip on the floor in Classroom 1. Manufacturer's documentation affixed to the unit did not indicate the maximum temperature achieved by the heating element for the unit. Based on interview at the time of the observations, the Director of Risk Management and the Plant Operations Director stated they did not know the temperature achieved by the heating element for the unit and agreed an operable portable space heater was noted at the aforementioned location.

Electrical Equipment - Power Cords and Extens

Based on observation and interview, the facility failed to ensure 1 of 1 extension cords including power strips were not used as a substitute for fixed wiring. LSC 19.5.1 requires utilities to comply with Section 9.1. LSC 9.1.2 requires electrical wiring and equipment to comply with NFPA 70, National Electrical Code, 2011 Edition. NFPA 70, Article 400.8 requires that, unless specifically permitted, flexible cords and cables shall not be used as a substitute for fixed wiring of a structure. This deficient practice could affect over 10 residents, staff and visitors in the vicinity of Classroom 1.
