Based on observation, the facility was storing medical gas cylinder(s) in a manner which was not in conformance with NFPA 101 Life Safety Code (2000 edition), Chapter 19, Section 19.3.2.4 and NFPA 99 Standard for Health Care Facilities (1999 edition) Chapter 4, Section 4-3.1.1.1. This deficient practice could adversely affect staff or visitors in the vicinity of the Medical Gas Storage Room, if one or more cylinders were to fall-over and rupture.

FINDINGS INCLUDE:

On 6/3/10 at 12:02 PM, observation revealed twelve (12) H-size oxygen cylinders stored within the Oxygen Storage Room. These cylinders were stored upright on the floor, and were not individually secured and located to prevent falling or being knocked-over. This free-standing storage arrangement was not in conformance with NFPA 99 (1999), Chapter 4, Section 4-3.1.1.1 and Chapter 8, Section 8-3.1.1.

This finding was confirmed with the chief building engineer [GK] at the time of discovery.

Generators must be inspected weekly and exercised under load for 30 minutes per month and shall be in accordance with NFPA 99 (1999 edition) and NFPA 110 (1999 edition).

NFPA 110 (1999 edition) Chapter 3, Section 3-5.5.6 requires the following: All Level 1 and Level 2 installations shall have a remote manual stop station of a type similar to a break-glass station located outside the room housing the prime mover, where so installed, or located elsewhere on the premises where the prime mover is located outside the building.

NFPA 110 (1999 edition) Chapter 3, Section 3-5.6.1 requires the following: A remote, common audible alarm powered by the storage battery shall be provided as specified in 3-5.5.2(d). This remote alarm shall be located outside of the EPS service room at a work site readily observable by personnel.

FINDINGS INCLUDE:

On 6/3/10 between 9:00 AM and 2:00 PM, it was learned the facility was not equipped with a remote, common audible alarm panel for the emergency generator.

This finding was verified with the chief building engineer (GK).

NFPA 101 (2000) LIFE SAFETY CODE SURVEY STANDARD - All existing escalators, dumbwaiters, and moving walks conform to the requirements of ASME/ANSI A17.3, Safety Code for Existing Elevators and Escalators, in accordance with NFPA 101 (00) Chapter 19, Section 19.5.3 and Chapter 9, Section 9.4.2.2.

FINDINGS INCLUDE:

On 6/3/10 at 12:33 PM, observation revealed a dumbwaiter connecting the basement with the main floor. In an interview with the chief building engineer (GK), it was learned the dumbwaiter was not equipped with hoistway door locking mechanisms which would keep the doors closed, except for the floor where the car would be loaded/unloaded. This arrangement was not in conformance with ASME/ANSI A17.3.
