Based on observation, staff interview and review of maintenance records on 11/30 through 12/01/2021, the Aurora St Luke's Medical Center failed to construct, install, and maintain the building systems to ensure safety of patients.
Findings include:
The facility was found to contain the following deficiencies.
K345 - Fire Alarm System Testing and Maintenance
K711 - Evacuation and Relocation Plan
A709 - Life Safety from Fire
As a result of these deficiencies, 42 CFR Subpart CFR 482.41 Condition of Participation: Physical environment was NOT MET
