Based on observations during the tour of the facility on the morning of 12/03/2015, with the Chief Compliance Officer and Director of Nursing and Director of Maintenance, the facility failed to maintain egress corridors. On the second floor in wing A, the linen closet had holes in its door, allowing smoke to enter into the egress corridor.



Based on observations during the tour of the facility on the morning of 12/03/2015, with the Chief Compliance Officer and Director of Nursing and Director of Maintenance, the facility failed to provide an acceptable means to protect the corridor. Patient room 213 did not latch at its doorframe when the door was in a closed position.

On the second floor in nursing wing B, the stairwell door did not completely latch in its frame.
