Findings include:
1. On 11/12/13 at 11:25 AM, in the presence of Staff #59, at Room #C119, the corridor door could not automatically close.

Based on observation, it was determined that the facility failed to provide a one hour rated construction for hazardous areas.
Findings include:
1. On 11/12/13 at 10:25 AM, in the presence of Staff #59, in Room #A213:
a. The wall between the storage room and the office was non rated.
b. There was a penetration in the corridor wall.

Based on observation, it was determined that, the facility failed to provide and illuminate exit signs at all exits.
Findings include:
1. On 11/12/13 at 1:45 PM, in the presence of Staff #59, in the C level corridor at C138, there was no exit sign.
2. On 11/13/13 at 2:15 PM, in the presence of Staff #59, in the G level corridor, there was no exit sign in front of the smoke door.

Based on observation, it was determined that the facility failed to maintain and inspect the sprinkler system.
Findings include:
1. On 11/12/13 at 10:35 AM in the presence of Staff #59, four boxes were stored within 18" of the sprinkler system in the shop.
2. On 11/13/13 at 11:20 AM in the presence of Staff #59, two boxes were stored within 18" of the sprinkler system in Room #D341.

Based on observation, it was determined that the facility failed to protect gas in storage in accordance with NFPA 99 4-3.1.1.1.
Findings include:
1. On 11/12/12 at 10:35 AM in the presence of Staff #59, four tanks of gas it were observed unsecured in the shop. NFPA 99 4-3.1.1.1 states cylinders in service and in storage shall be secured and located to prevent falling or being knocked over.
