Based on record review and staff interview the facility fails to conduct fire drills at least quarterly on each shift. This deficient practice could result in staff being unfamiliar with proper procedures to respond and react to a fire incident affecting 5 of 5 smoke zones. The facility has a capacity of 25 with and census of 12 at the time of this survey.

Findings include:

During the survey on 12/06/2010 between 9:30 am and 2:00 pm the following was observed:

1. There is no documentation showing a fire drill was conducted on the 1st shift during the 4th quarter of 2009 or 2010.

2. There is no documentation showing a fire drill was conducted on the 3rd shift during the 3rd quarter of 2010.

Staff A and B were present and confirmed there was no documentation for the shifts and quarters indicated above.

This deficient practice could result in delayed egress from the facility in the event of an emergency affecting 1 of 5 smoke zones. The facility has a capacity of 25 and a census of 12 at the time of this survey.

Findings include:

During the survey on 12/06/2010 between 9:30 am and 2:00 pm it is observed that there is a gurney and table in the corridor outside the Dietary Department.

Staff A and B were present and confirmed the items were in the corridor.

This deficient practice could result in personal being unable to access the electrical panels in a timely manner affecting 1 of 5 smoke zones. The facility has a capacity of 25 and a census of 12 at the time of this survey.

Findings include:

During the survey on 12/06/2010 between 9:30 am and 2:00 pm it is observed that there is storage in front of an electrical panel in the mechanical room on the south end of the second floor.

Staff A and B were present and confirmed that there was storage in front of the electrical panel.
