Dead-End Corridors and Common Path of Travel

Based on observation during the survey walk-through, a smoke compartment contains one means of egress. This deficient practice could affect the safe egress of all patients, staff and visitors within that smoke compartment if a fire incident occurs in the adjacent compartment.

The finding is:

On 12/06/2016 at 10:55am while accompanied by the DE it was observed that the second floor Patient care unit has exiting in only one direction which does not comply with 19.2.5.2 for dead end corridors exceeding 30 feet in length.

Exit Signage

Based on observation during the survey walk-through, exit signs were not provided, were not fully visible, or incorrectly identified paths of egress. These deficiencies could affect all patients within the areas of the facility, as well as any staff and visitors present, by preventing those occupants from readily identifying the path to an available exit from the building.

The finding is:

A. On 12/06/16 at 10:15pm while accompanied by the DE it was observed that only one path of exit access was identified by exit signage which does not comply with 19.2.5.4. Locations observed include the following:

1. 2nd floor corridor located adjacent to Patient Observation rooms.

2. 2nd floor horizontal exit located adjacent to tne nurse station, which leads to the South stair and ICU suite.

Fire Alarm System - Initiation

Based on observation during the survey walk-through, not all portions of the building fire alarm system are installed in accordance with Code requirements. This deficiency could affect any patients, staff, or visitors in the immediate area by causing the smoke detector to fail to operate under fire conditions.

The finding is:

On 12/06/2016 at 11:45am, while accompanied by the DE, in the lower level the West exit stair was observed which lacked a manual pull station within the means of egress corridor to the Stair entry door. This does not comply with NFPA 72.

On 12/06/2016 at 11:55am, while accompanied by the DE, in the lower level West exit stair a pull station was observed within the stair. This pull station is for exiting from the Mechanical room, however, the pull station is on the wrong side of the wall. The pull is to be activated prior to stair access. This does not comply with NFPA 72.
