Based on interview and record review, the facility failed to provide 2 (#6, #7) of 10 Surgical Out-Patients with adequate medication allergy precautions. Findings include:
Patient #6 had only egg allergies identified on the "Admission Record / Patient History" and eggs and sulfa allergies documented on the "Post Anesthesia Record" of 5/8/12. The patient's "Oral Surgery Pre-Operative Orders" dated 4/19/12 and reviewed on 5/8/12, documented "Allergies: Antibiotics, General Anesthesia, Iodine, Codeine, Sulfa" these were not readdressed and updated in the out-patient surgical documentation forms. Interview with the Director of Surgery, on 7/2/12 at approximately 1200, verified that no one had updated the information. The patient was given a prescription for Lortab that was filled at the facility out-patient pharmacy as verified by the Pharmacy Tech and Pharmacy Manager on 7/2/12 at approximately 1000. The pharmacy dispensed the Lortab although this medication was contraindicated with a Codeine allergy. The out-patient pharmacy listed no allergies. This lack of updating surgical information and systems communication between surgery and pharmacy posed a significant potential for preventable adverse drug reactions.
Patient #7 also had out-patient surgery on 5/8/12 and the History and Physical listed a Prednisone allergy with nausea/vomiting. All the Out-Patient Surgery forms listed NKDA (no known drug allergies). Further interview with the Surgical Director verified the H&P is required before surgery but that no one documented addressing the discrepancy.
