Based on a tour of the facility it was noted that there were isolated areas in the mechanical room areas where the spray applied fireproofing, was damaged or missing and needed to be patched. ( 19.1.1.4.1) There were also isolated penetrations in the electrical and utility closet ceilings that were not sealed in an approved manner.

Based on observations made during a tour of the facility it was determined that the coordinating device on a pair of fire doors to the passageway leading to the Rosenberg Building were not adjusted or maintained in order for the doors to close in proper sequence. (19.2.2.2.6)

Based on observations made during a tour of the hospital it was determined that:
1. A power operated sliding door at the main hospital entrance has emergency breakaway features. A metal threshold installed at the doors prevented the doors from swinging open . ( 19.2.1)
2. Delayed egress hardware was installed on several exit doors. One of the stairway discharge doors lacked required signage reading " Push Until The Alarm Sounds - Doors Can Be Opened In 15 Seconds." (19.2.1)
3. Magnetic locking devices have been installed on the two exterior exit doors from the cafeteria. The occupant load shall be posted for and limited to 49 persons or provisions shall be made to release the lock at the second exterior door. (19.2.1)

Based on observations during a tour of the hospital it was determined that;
1. An illuminated exit sign is needed at the center stairway exit discharge door.
2. Egress is identified through an office in the Rosenburg Building and difficulty was encountered opening the exterior door . Other approved exits were identified, therefore, exit signage identifying egress through the office must be eliminated. ( 19.2.10.1)

Based on the observations made during a tour of the hospital it was determined that:
1. Additional sprinklers are needed in room E2-226 to provide adequate coverage . (19.3.5)
2. The chlorine damaged sprinkler required replacement in the hydro therapy pool area. (19.3.5)
3. Sprinkler wrenches were not provided in each sprinkler cabinet. ( 19.3.5)

Based on a tour of the facility it was determined that a minimum 2A10BC portable fire extinguisher shall be provided in the rehabilitation apartment D1-109 to serve the cooking range. ( 19.3.5.6)
