Based on observation, the facility failed to ensure exit access was readily accessible at all times. The findings were:

Observation of equipment storage (#513) on 02/25/14 at 1:37 PM showed the (2) doors swung into the exit access corridor and were restricted from complete opening by overhead closers. The resulting fully opened doors protruded more than 7 inches into the required width of the corridor.

Reference NFPA 101, 2000 Edition,
18.2.1 General. Every aisle, passageway, corridor, exit discharge, exit location, and access shall be in accordance with Chapter 7, unless otherwise modified by 18.2.2 through 18.2.11.

7.2.1.4.4* During its swing, any door in a means of egress shall leave not less than one-half of the required width of an aisle, a corridor, a passageway, or a landing unobstructed and shall project not more than 7 in. (180 mm) into the required width of an aisle, a corridor, a passageway, or a landing, when fully open, unless both of the following conditions are met:
(1) The door provides access to a stair in an existing building.
(2) The door meets the requirement that limits projection to not more than 7 in. (180 mm) into the required width of the stair landing when the door is fully open.

Based on observation, the facility failed to provide full coverage by the automatic sprinkler system . The findings were:

Observation of dry storage (#184) in the kitchen on 02/25/14 at 2:19 PM showed a pendant sprinkler head was located 2 inches from a surface-mounted light fixture that protruded down 3 inches. The sprinkler head protruded down approximately 1 inch.

9.7 Automatic Sprinklers and Other Extinguishing Equipment.
9.7.1 Automatic Sprinklers.
9.7.1.1* Each automatic sprinkler system required by another section of this Code shall be in accordance with one of the following:
(1) NFPA 13, Standard for the Installation of Sprinkler Systems
(2) NFPA 13D, Standard for the Installation of Sprinkler Systems in One and Two-Family Dwellings and Manufactured Homes
(3) NFPA 13R, Standard for the Installation of Sprinkler Systems in Residential Occupancies up to and Including Four Stories in Height

8.6.5.2.1 General.
8.6.5.2.1.1 Continuous or noncontinuous obstructions less than or equal to 18 in. (457 mm) below the sprinkler deflector that prevent. the pattern from fully developing shall comply with 8.6.5.2.
8.6.5.2.1.2 Regardless of the rules of 8.6.5.2, solid continuous obstructions shall meet the applicable requirements of 8.6.5.1.2.

Based on observation, the facility failed to ensure portable fire extinguishers were properly installed. The findings were:

Observation of the K-type fire extinguisher in the kitchen on 02/25/14 at 2:21 PM showed the fire extinguisher did not have recommended markings that indicated the extinguisher's suitability according to the class of the fire.

B.1 General.
B.1.1 Markings should be applied by decals that are durable and resistant 1.0 color fading. The color separation identification for the markings is as follows:
(1) Picture symbol objects are white.
(2) Background borders are white.
(3) Background for "YES" symbols is blue.
(4) Background for symbols with slash mark ("NO") is black.
(5) Class of fire letters and wording are black.
(6) Slash mark for black background symbols are red. (See Figure B.I.I.)
