Based on observation and interview, it was determined the fire protection of the Soiled Linen Room was compromised by missing door hardware, which allowed for passage a fire and smoke through the opening, and a damaged door that prevented the door from fully closing as required. The failed practice had the potential to affect the health and safety of all patients, staff, and visitors due to the potential spread of smoke and fire due to the opening left by the missing door hardware and inability of the door to close due to the damaged door. The facility had a census of two patients on 01/11/16. The findings follow:
A. On a tour of the facility with the Maintenance Director on 01/15/16 at 0930, the door to the Soiled Linen Room was observed with missing door hardware. The missing hardware left an opening in the door to allow passage of smoke and fire through the hole in the door. The door had also pulled away from the door hinges, which caused the door to not fit within the door frame and prevented the door from closing fully.
B. The Maintenance Director verified the missing door hardware and damaged door at the time of the inspection of the Soiled Linen Room.
