Based on observation , interview, and record review; the facility failed to maintain the surgical flooring free from gaps between the pieces of linoleum; holes on the mopboard and corners of the surgical cabinets located in three operating suites (the minor procedure room, endoscope room (tubes inserted either in mouth down throat though esophagus into stomach or; up through rectum into the colon); and the instrument processing room) and in hallways throughout surgical areas. These gaps and holes prevented a thorough clean and are a potential cause for infection control issues for all surgical patients.

Findings are:

A. Review of the Surgical Area on 11/6/19 at 9 AM revealed:
- The hallways contained multiple separation along the linoleum floor seam varying in sizes from approximately 2 inches to 6 inches.
- The Minor Procedure room had multiple separations along the linoleum floor seams varying in sizes.
- The Endoscopy room had multiple separations along the linoleum floor seams varying from 2 inches to 6 inches.
- OR 1 had major separations on all the linoleum floor seams varying from 2 inches to 24 inches. The stock supply cabinet had holes in the linoleum in the corners. There were two rips in the linoleum along the mopboard and indentations in the flooring.
- OR 2 had separations on all the linoleum floor seams varying from 2 inches to 6 inches. The stock supply cabinet had holes in the linoleum in the corners.
- OR 3 had separations and scuffing on all the linoleum floor seams varying from 24 inches to 48 inches. The mopboard made out of flooring was rough and contained a small piece of a red cloth.
- The repaired area of the linoleum floor seams had areas of separation.

B. Interview with the Director of the Operating Room, on 11/6/19 at 10 AM, revealed that the flooring in the surgical area was approximately 5 years old. The facility had attempted to repair the flooring seams but they were separating.

C. Review of the Quality Assurance Minutes revealed that the Surgical Flooring continued to be an issue with no resolution or intervention.
