Based on observation and interview, the facility failed to maintain the fire sprinkler system. This has the potential of affecting approximately 4 occupants in one of ten smoke zones by preventing the sprinkler system from proper activation in the case of a fire. This facility has a capacity of 25 and census of patients 10 at the time of the survey.

Findings are:

On 11/5/2019 at 1:15 P.M., observations revealed that the facility failed to provide the required 18 inch clearance to obstructions from the automatic fire sprinkler for the storage room 1A116. The items stored on the portable shelf are within the required 18 inches of the fire sprinkler.

During interview conducted on 11/5/2019 at 1:15 P.M., Maintenance personnel stated that they were unaware of the items stored within the required clearance to the sprinkler head.
