Based on observation and record review, the facility failed to maintain the automatic sprinkler system.

The findings include:
Observation and record review on 12/11/17 at 11:45 AM revealed no 3 year air leakage test has been conducted on the dry sprinkler system.
Maintenance was present when the deficiency was identified and was acknowledged by the Director of Clinical Services during the exit conference on 12/11/17.

Based on observation, the facility failed to provide a precautionary sign on the door of the oxygen cylinder room.

The findings include:
Observation on 12/11/17 at 2:00 PM revealed the door to the oxygen storage room and manifold room are not provided with a sign that includes the following wording as a minimum, CAUTION: OXIDZING GAS(ES) STORED WITHIN NO SMOKING.
Maintenance was present when the deficiency was identified and was acknowledged by the Director of Clinical Services during the exit conference on 12/11/17.
