Based on observation and interview the facility failed to provide protection from the extremes of the weather and from the ground beneath to prevent rusting.

During a survey of the facility on 3/22/2010 at 12:08 it was observed that the oxygen cylinders that where stored outside were not protected from the extreme weather elements. and they were not protected from the ground beneath to protect from rusting.

During an interview of the maintenance supervisor on 3/22/2010 at 12:10 it was revealed that they didn ' t know the oxygen cylinders had to be protected from the elements.

The census of 7 and a capacity of 25 beds was acknowledged and verified by the maintenance supervisor during the exit interview on 3/22/2010.
