Based on interview and record review the facility failed to ensure that facility staff were educated on first aid response techniques related to restrained patients, for six staff (B, D, E, G, M, U) personnel files of six staff personnel files reviewed. This failure had the potential for serious injury or death to any patients that required restraints in the facility. The facility census was 87.
Findings included:
1. Record review of six staff personnel files Staff B, Registered Nurse (RN); Staff D, RN, Chief Executive Officer; Staff E, Director of Nursing; Staff G, RN, Nurse Manager; Staff M, Therapist; and Staff U, Director of Facilities, showed no evidence of first aid education.
2. Record review of the facility's policies and procedures showed no evidence that a policy or procedure existed to ensure regulatory compliance for first aid in restraint education.
During an interview on 06/16/16 at 2:45 PM, Staff E, Director of Nursing, stated that there was no specific training for anyone in the hospital for first aid in restraints, including physicians, and no facility policy and procedure.
