Based on observations and interview during facility tour with maintenance staff of George E Weems Memorial Hospital, the Facility failed to maintain Fire and Smoke doors with required rating labels. Non rated fire and smoke doors compromise the integrity of the fire wall or smoke compartment allowing penetration of fire, heat, smoke, and toxic gases endangering all occupants.
The findings include:
During the facility tour with maintenance staff on 2/12/2013 at 11:25am, it was found that all Fire/Smoke doors and frames inspected were missing rating certification tags. The maintenance staff stated that the doors had been ordered and that the facility was waiting on delivery and installation. According to NFPA 101 2000 edition 19.3.6.3.7 "Door frames shall be labeled, shall be of steel construction, or shall be of other materials in compliance with provisions of Section NFPA 101 2000 edition 8.2.3.2.3."
