Based on observation and interview, it was determined the Emergency Department (ED) failed to ensure expired supplies were not available for patient use in two of two ED rooms surveyed. The failed practice did not ensure only current supplies were provided for patient care and created the potential to affect any patient in the ED. Findings follow.
A. During a tour of the ED on 06/03/15 from 0840 to 0900 the following expired items were observed:
1) Exam room - 10 of 10 Cotton Tipped Applicators - expired 03/2015
2) Trauma room - 15 of 15 Cotton Tipped Applicators - expired 03/2015; 4 of 5 blue top vacutainers expired 01/2015, 02/2015, 04/2015 and 05/2015.
B. During an interview on 06/03/15 at 1045, when asked if there was a policy regarding expired supplies the Director of Nursing said there was not.
C. The expired items were confirmed by the ED Director at the time of the tour.
