Based upon observations, interviews, and record review, the hospital failed to ensure ligature hazards were not present. Findings:
Observations made during the environmental tour on 9/22/15 at 10:10 a.m. revealed the following ligature hazards were present in rooms #1 through #7: 1) all the patient room doors and bathroom doors had three separate hinges on the door jams with approximately a 24 inch gap between the hinges; 2) The bathroom sinks in all patient rooms had goose neck spouts and flange hot and cold handles. 3) The pipes under the sinks in all patient bathrooms were exposed.
Interview on 9/22/15 at 11:55 a.m. with SF1Administrator and SF2Director of Nursing revealed the hospital had ordered door hinges but they had not arrived as of 9/22/15. When asked about the faucets, SF1Administrator replied they were still getting bids from plumbing companies for the faucets and instillation.
Review of the written quotes obtained for anti-ligature hinges revealed the expected arrival dates were 9/23/15 and 9/30/15. Also provided for review were two quotes from contractors for the lavoratory equipment and installation and dated 8/28/15 and 9/18/15. Interview with SF1Administrator on 9/22/15 at 1:50 p.m. revealed they would hopefully have an answer from corporate by the end of the week on which contractor would be used.
