Based on observations, the facility failed to maintain the exit access.
The findings included:
1. Observation on 12/18/2015 at 09:25 AM, revealed latching and locking hardware requiring more than one releasing operation on patient room doors on the 5th floor in the following locations:
a. A- wing (rooms 500-520)
b. B- wing (rooms 521-540)
c. C wing (rooms 541- 564)
NFPA 101, 7.2.1.5.4 (2000 Edition)
2. Observation on 12/22/2015 at 09:40 AM, revealed dead bolt style locks installed above 48 inches on patient room doors on the 5th floor in the following locations:
a. A- wing (rooms 500-520)
b. B- wing (rooms 521-540)
c. C- wing (rooms 541- 564)
NFPA 101, 7.2.1.5.4 (2000 Edition)
These findings were acknowledged and verified by the Facilities Manager during the facility tour on 12/18/2015 and 12/22/2015.
