Based on observation and staff interview, it was determined that the facility failed to comply with NFPA 101 (2000) edition. 19.5.2.1, 9.2.1, NFPA 90A (1999). This deficiency could affect all occupants of the facility in case of a fire or other emergency.

Findings include:

During the life safety tour of the facility on 02/24/2014 at 2:55 PM with the Director of Facilities, it was observed that an exhaust fan was not operational on the 1st floor East in the west corridor environmental closet in the Lake Pavilion across from social work service room.

During the staff interview on 02/24/2014 at 2:58 PM, the Director of Facilities acknowledged that an exhaust fan was not operational on the 1st floor East in the west corridor environmental closet in the Lake Pavilion across from social work service room.

Major appliances and patient equipment must be plugged directly into wall receptacle. This deficiency could affect all occupants of the facility in case of a fire or other emergency.

Findings include:

During the life safety tour of the facility at 12:10 PM on 02/26/14 with the Director of Facilities, it was observed that a microwave was plugged into a surge protector in the Nurses's Lounge in the Tower Building on the 4th floor.

During the staff interview at 12:12 PM on 02/26/14, the Director of Facilities acknowledged that a microwave was plugged into a surge protector in the Nurses's Lounge in the Tower Building on the 4th floor.
