During the recertification survey, based on observations and staff interviews, the facility failed to ensure that the doors protecting corridor openings were maintained in accordance with NAPA 101 18.3.6.3.
Findings include:
Observation on 02-20-14 revealed that:
1. The 20 minute rated fire door to room 226 would not latch when tested.
2. The 20 minute rated fire door to the nurses station had damaged hardware and would not latch when tested.
3. The double doors to the maintenance shop only had a deadbolt and would not achieve a positive latch when tested.
The above items were verified by the Director of Engineering at the time of discovery.

On the day of the Recertification survey, based on observations, record review, and staff interview, the facility failed to ensure that the automatic sprinkler system was maintained in accordance with NFPA 25.
Findings include:
Record review on 02-20-14 revealed that there was no documentation to show that a five year obstruction investigation has been completed. Also there was no documentation to show that the gauges have been replace or calibrated with in the last five years.
The above items were verified by the Director of Engineering at the time of discovery.
