During the Life Safty Code Survey conducted on 01-09-19,Greenleaf Center is in compliance with the requirements of 42 CFR Part 482,Subpart C requirements for Health Care Facilities.

Based on observation,and staff interviews it was determine the facility to ensure that the electrical panels were properly labeled and all Multi tap on floor were properly secured.
This could place 30 % resisents at risk in the event of an emergency.
The findings include:
During a tour of the facility with Staff M on 01-09-19 between 10:00 am and 3:00 pm the facility failed to ensure that electrical panels were properly labled throughout the facility and that all multi tap were attach to the wall or two the furniture throughout the facility.
These findings were confirmed by Staff M at the time of discovery.

Fire Drills

Based on observation and review of facility records it was determine the facility failed to ensure that Fire Drills were condcuted as required.
This could place 50 % residents at risk in the event of an emergency.
The findings include:
During a tour of the facility with Staff M on 01-09-19 between 10:00 am and 3:00 pm records revealed that fire drills were not completed per shift per quarter,Jan,March 7 TO 7 PM and 3 to 11.
These findings were confirmed by Staff M at the time of discovery.
