Based on observation, the facility failed to provide the two-hour fire separation between changes of occupancy in accordance NFPA 101 section 19.1.1.4.4. This deficient practice has the potential of affecting the entire facility.
Findings Include:
While inspecting the hospital on 06/19/15 at 11:00 a.m., observation revealed no two-hour fire wall separation with 90 minute fire doors between the hospital and a leased space for an outpatient's clinic.
The finding was acknowledged by the Administrator and verified by the Maintenance Supervisor during the exit interview on the exit interview on 06/19/15.
19.1.1.4.4 Changes of Occupancy.
Changes of occupancy shall comply with 4.6.11. A change from one health care occupancy sub classification to another shall require compliance with the requirements for new construction.

Health care occupancies in buildings housing other occupancies shall be completely separated from them by construction having a fire resistance rating of not less than 2 hours as provided for additions in 18.1.1.4.
4.6.11* Changes of Occupancy
In any building or structure, whether or not a physical alteration is needed, a change from one occupancy classification to another shall be permitted only where such a structure, building, or portion thereof conforms with the requirements of this Code that apply to new construction for the proposed new use or, where specifically permitted elsewhere in the Code, existing construction features shall be permitted to be continued in use in conversions.

Based on observations the facility failed to provide the required readily accessible exit access in an existing facility in accordance NFPA 19.2.1, 7.1. This deficient practice has the potential of affecting two (2) of four (4) smoke compartments of the facility.
Findings Include:
While inspecting the means of egress in Zone 1 of the facility on 06/19/15 at 10:35 a.m., observation revealed two (2) dead-end corridors on the Nursing South End. The Nursing South End is under construction and staff/patients are not allowed to use the exit.
The finding was acknowledged by the Administrator and verified by the Maintenance Supervisor during the exit interview on the exit interview on 06/19/15.
