Based on observation, the facility failed to maintain and seal smoke/fire wall penetrations in 3 of 8 smoke/fire walls in the facility.

Findings include:

The following smoke/fire barrier locations had unsealed penetrations:
.
1) Entering the 100 unit, above the ceiling tiles above the door, there was an one-inch flexible conduit penetrating the smoke/fire wall that was not sealed against the passage of smoke.

2) Entering the 400 unit, above the ceiling tiles above the door, there was an one-inch flexible conduit penetrating the smoke/fire wall that was not sealed against the passage of smoke.

3) In the main corridor entering the 400, 500 and 600 wing, above the ceiling tiles above the cross corridor doors, was an one-inch hole in the smoke/fire wall that was not sealed against the passage of smoke.

NFPA 101
Chapter 12-5.1.1 Utilities shall comply with the provisions of Section 7-1
Chapter 7-1.2 Electrical wiring and equipment installed shall be in accordance with NFPA 70, National Electric Code.
Exception: Existing installations may be continued in service subject to approval by the authority having jurisdiction.


EXTENSION CORDS:
NFPA 70, Section 400-8 Unless specifically permitted in Section 400-7, flexible cords and cables shall not be used for the following:
(a) As a substitute for fixed wiring of a structure
NFPA 70, Section 305-4(h) Protection from Accidental Damage. Flexible cords and cables shall be protected from accidental damage. Sharp corners and projections shall be avoided. Where passing through doorways or other pinch points, protections shall be provided to avoid damage.

Based on observation, the facility was using extension cords as permanent electrical wiring.

Findings include:

1) In the 200 nurses station area was a computer mounted on the wall. Two surge protectors (approximately 3 foot long) plugged into each other were being used as permanent wiring to supply power to the computer.

2) In the main mechanical room across from the kitchen, the facility was using an extension cord as permanent wiring to supply power to the building's water softener.
