Based on observation and interview it was determined the facility failed to maintain exit stair towers affecting one of nine smoke compartments within the facility.

Findings include:

1. Observation on October 31, 2016, at 11:15 AM revealed the 2nd floor exit stair tower had an unsealed penetration around a sprinkler pipe.

Interview with maintenance representative #1 on October 31, 2016, between 1:15 PM and 1:20 PM confirmed the penetration.

Interview with maintenance representative #1 on October 31, 2016, between 1:15 PM and 1:20 PM confirmed the lack of signage.

Interview with maintenance representative #1 on October 31, 2016, between 1:15 PM and 1:20 PM confirmed the unsecured cylinders.
