Based on observation and interview it was determined the facility failed to maintain exit stair towers affecting one of nine smoke compartments within the facility.

Findings include:

1. Observation on October 31, 2016, at 11:15 AM revealed the 2nd floor exit stair tower had an unsealed penetration around a sprinkler pipe.

Interview with maintenance representative #1 on October 31, 2016, between 1:15 PM and 1:20 PM confirmed the penetration.

Based on observation and interview, it was determined the facility failed to install and maintain illuminated exit signage in one instance affecting two of nine smoke compartments within the facility.

Findings include:

1. Observation on October 31, 2016, at 12:00 PM revealed the lack of exit signage above the cross corridor doors near the Lab.

Interview with maintenance representative #1 on October 31, 2016, between 1:15 PM and 1:20 PM confirmed the lack of signage.

Based on observation and interview it was determined the facility failed to store medical gas cylinders affecting one of nine smoke compartments within the facility.

1. Observation on October 31, 2016, at 12:30 PM revealed four unsecured "E" sized oxygen cylinders located in the 1st floor (facility basement) O2 storage room.

Interview with maintenance representative #1 on October 31, 2016, between 1:15 PM and 1:20 PM confirmed the unsecured cylinders.
