Tag No.: K0032
Based on observation, on December 15, 2009 at approximately 11:00am onward, the means of egress is incomplete in the following areas:
a. stair #3, stair #4, and center stairwell ( Patient tower lower level)- exit discharges are incomplete to the publicway with solid slip-resistant surfaces and emergency lighting.
b. lower level rehab and central stair terminal - existing travel distances exceed 150 feet in a single direction measured from the center of arranged corridors.
c. security office in room 1B216 - facility does not have a wiring diagram posted adjacent to the main fire alarm control panel for the existing special locking arrangement.
d. ER - special locking arrangement did not release during activation of the facility fire alarm system at approximately 2:10pm on December 16, 2009. This deficiency was abated on December 17, 2009 during a retest of the facility fire alarm system. There is no master release switch at ER nurses' station .
e. Exit discharge door/gate from Behavoiral Health courtyard had slide bolt to secure door.
Follow-up survey 2/18/2010 - All K-32 Tag items corrected by 1/17/2010 except K-32 a. which is waivered.
