During an interview on 6/28/13 at 11 a.m., the pharmacy director confirmed the facility did not currently have a policy related to responsibility for evaluation of external alerts and/or recommendations from national associations for medication alerts requiring attention and or review.

Based on record review and interview, the facility did not have a policy for automatic stop dates for specific categories of medications.

Findings:

During an interview on 6/28/13 at 11:30 a.m., the pharmacy director confirmed the facility did not currently have a policy for automatic stop dates for specific categories of medications. He said the software currently just defaults to end the medications in 330 days from the start of the medication and does not consider the category of the drug.
