Based on observations made during the survey of the facility between the hours of 9:00 am and 5:00 pm (Day 1), and between the hours of 8:30 am and 1:00 pm (Day 2), while accompanied by the Interim CEO, Dir. Of Support Svcs., Mgr. Of Support Svcs., Chief Nursing Off., and Plant Ops Mgr. it was observed that the facility was not in compliance with the following:

As REHAB PATIENTS utilize the required fire exit doors (a single pair of exit doors) leading them outside from the interior of the REHAB GYM to the exterior PUBLIC WAY the configuration of the egress components located immediately outside of these doors is not conducive for moving patients safely away from the building. PUBLIC WAY means a sidewalk, alley, street, highway, square or another open space to which the public has access, as of right or by invitation, expressed or implied.

These doors swing outward and if opened only 90 degrees effectively block off any egress movement - which could happen with REHAB PATIENTS who might not have sufficient mobility and strength. The reason occupants leaving the Gym via this fire exit route are blocked from continuing on a straight pathway forward is due to placement of a steel guard railing located roughly 42 inches or so away from the building. Occupants must swing the doors 180 degrees and turn to the right, or to the left to continue movement towards the PUBLIC WAY. As occupants turn either way they are quickly met with an end to the concrete stoop, and the pathway towards a safe area away from the building is uneven, poorly graded soil and grass areas whose slopes greatly exceed the maximum safe slope.

An effective "means of egress" is required all the way to the PUBLIC WAY. This is not currently provided.

Provide an exterior pathway which allows REHAB PATIENTS and other occupants to safely, easily, and quickly move a safe distance away from the building in the event of a fire/smoke occurance in this facility.

MISSING SPRINKLER HEADS AT COLD STORAGE (Walk-in Units): NFPA, 1994: 3-8 "Cold Storage Rooms."

Regarding: Cecil R. Bomar Rehabilitation Center (Hosp. 2 - Bldg. 2):
Based on observations made during the survey of the facility between the hours of 9:00 am and 5:00 pm (Day 1), and between the hours of 8:30 am and 1:00 pm (Day 2), while accompanied by the Interim CEO, Dir. Of Support Svcs., Mgr. Of Support Svcs., Chief Nursing Off., and Plant Ops Mgr. it was observed that the facility was not in compliance with the following:
The small emergency lp gas-fired generator was located in a small room that did not provide the required 2-hour fire separation from the rest of the building. Additionally, there were wall mounted electrical devices including an automatic transfer switch and an electrical distribution panel.
