Based on review of the facility policy, observation, and interview, the facility failed to ensure expired intravenous (IV) medical supplies were discarded in 1 of 1 pediatric crash carts reviewed.

The findings include:

Review of the facility's policy titled "Expired Medical Supplies - Ballad Health," dated 1/24/2020, showed "...only use medical supplies that are in good condition and are within the manufacturers expiration date..."

Observation and interview on 1/19/2022 at 2:05 PM, in the Emergency Department (ED), with the Director of Case Management/Clinical Coordinator and Risk/Quality Manager revealed the ED pediatric crash cart contained fifteen 24-gauge angiocath needles (used for IV site access) with an expiration date of 9/14/2021 and two IV start kits with an expiration date of 10/30/2021. The Director of Case Management/Clinical Coordinator confirmed the angiocath needles and the IV start kits were expired and available for patient use.
