Based on observation on 02/27/2013 the following rooms were being held open by wedges, blocks, chairs and other items;
a. Visitor's Lounge D2004
b. Cart Wash N 1034
c. Third (3) floor mail room
d.) 0 floor door back of the kitchen and grill.

Based on observation on 02/27/2013 the rated chase door N 2102 and others failed to close and latch.

A. Storage rooms B0031did not close and latch.
B. N1020 did not close (no closer on the door)
C. N1025 was wedged in the open position.
D. Resident rooms A 2015 and A 2021were converted to storage rooms and had no closers and did not close and latch .
E. The door to room N 1020 failed to close and latch.
F. The door to the kitchen storage Room N1008 failed to to latch when closed

A. Based on observation on 02/26/2013 the Exit at door B0084 did not have a hard surface path to a public way.

Based on observation on 02/27/2013 the exit from the Lab. had only one (1) directional sign for exiting .the Lab. area.

Based on observation on 02/27/2013 the 02 bank cover room E 1039 is attached to the building and has a combustable roof , this area is not covered by the sprinkler system.

A. Based on observation on 02/27/2013 the door to room N 3002 opens into the corridor , the door does not open 180 degrees nor does it have a closer. This door reduces the width of the egress corridor.
B. Based on observation on 02/27/2013 there were lockers stored in the corridor on the 0 level.
C. Based on observation on 02/27/2013 there were chairs stored in the corridor of the BO unit.
D. Based on observation on 02/27/2013 the corridor doors (3) for the Tulip class room #4 opened into the egress corridor , reducing the corridor width. There are two (2) other class room areas similar to this one.

A. Based on observation on 02/27/2013 there were full and empty 02 cylinders mixed in room F 0219.
B. Based on observation on 02/27/2013 the 02 cylinders in room E 1039 were not individually secured and the cylinders were resting on a concrete slab.
C. Based on observation on 02/27/2013 there was no signage indicating Full and Empty cylinders in room E 1039.

A. Based on observation on 02/27/2013 there were receptacles in the toilets and other rooms that were not identified as GFCI protected.
B. Based on observation on 02/27/2013 the Med. refrigerator in room A0006 was not connected to the emergency circuit.
