Based on observation and staff interview it was determined the facility failed to ensure the two hour fire resistance rated construction of the common wall with a nonconforming building was free of any penetrations. Findings:

On 11/04/15 at 9:45 a.m., the 2 hour fire barrier wall seperating the health occupancy from the business occupancy was obseved to have 7 penetrations. It was also observed that a white material was used to seal the gaps in the 2 hour fire wall barrier.

At 9:47 a.m., the supervisor of maintenance was interviewed. He acknowledged the penetrations in the fire wall.

Based on observation the facility failed to maintain their automatic sprinkler systems in reliable operating condition. FIndings:

On 10/04/15 at 11:55 a.m., a sidewall sprinkler head located in the lab room was observed to have paint on it.

At 11:58 a.m., the supervisor of maintanence acknowledged the paint on the sprinkler head.

Based on observation it was determined the facility failed to ensure no extension cords were being utilized within the facility. Findings:

On 11/04/15 at 11:43 a.m., an approximate 15 feet extension cord was observed in office A153. A power tap was plugged into the 3 hub extension cord.

At 11:43 a.m., the maintanence supervisor acknowledged the extension cord.
