Based on observation the facility failed to ensure that one of eight, cross-corridor doors in an exit passageway was held open only by an automatically closing device.

Findings include:

On 11/2/11, at 3:00 PM it was noticed that "convenience" doors in the Adolescent/Children's Hallway (near room 305), were held open with mechanical door stops installed at the base of each door.

NFPA 25, 2-2.1.1 Sprinklers shall be inspected from the floor level annually. Sprinklers shall be free of corrosion, foreign materials, paint, and physical damage and shall be installed in the proper orientation. Any sprinkler shall be replaced that is painted, corroded, damaged, loaded, or in the improper orientation.

This STANDARD was not met as evidenced by:

Based on observation, the facility failed to ensure that covers on two, pop-down, fire sprinkler heads could release from the assembly upon system activation.

Findings include:

On 11/2/2011, at 11:00 AM the cap covering the pop-down sprinkler head in the corridor restroom, in the 300 Hall, had been painted over and was adhering to the surface of the ceiling. At 11:30 AM the cap covering the pop-down sprinkler head in the Quiet Room in the Adult, Southwest section of the facility was also observed to be painted over and adhering to the surface of the ceiling.

NFPA 110, 5.6.6 Remote Controls and Alarms. A remote, common, audible alarm shall be provided as specified in 5.6.5.2 (4) that is powered by the storage battery and located outside of the EPS service room at a work site observable by personnel.

This STANDARD was not met as evidenced by:

Based on observation and staff interview, the facility failed to provide a remote alarm for the emergency generator at a work site where it would be readily observable by personnel during all shifts.

Findings include:

On 11/2/11, at approximately 11:45 AM, it was observed that the only monitoring panel for the 50 KW Cummings diesel generator was outside, and part of the generator set. The Director of Plant Operations confirmed that there was no monitoring panel inside of the facility.
