Based on observations, the facility failed to properly protect hazardous areas as required by NFPA 101 section 19.3.2.1. This deficiency practice affected the entire facility on the day of survey.
Findings include:
During the building inspection on August 23, 2018 at 11:10 AM, observation revealed the two (2) entrance doors to the Lab lacked automatic closing devices (door closures). The Lab was incapable of resisting the passage of smoke throughout the facility.

Based on observations and testing, the facility failed to properly maintain door openings in smoke barrier walls as per NFPA 101 section 19.3.7.8. This deficiency practice affected the entire facility on the day of survey.
Findings include:
On August 23, 2018 at 3:00 P.M., observation revealed to smoke barrier doors on the Pharmacy, Mop Room and Accounting Department lacked automatic closing devices (door closures). The doors to the Pharmacy and Mop Room did not meet the standard of smoke barrier doors. The door to the Mop Room contain air transfer grille and the Pharmacy door was made into Dutch door. These doors were incapable of resisting the passage of smoke throughout the facility.
