Based on record review and interview, the facility failed to document re-education/training of staff and house supervisors based on the recommendations of their investigation of 1 of 1 event on 11/02/2022.
Findings
The 11/02/2022 event was investigated and recommended making sure future events were escalated to the Administrator on Call, Security notified, and the block the room to prevent cleaning the room.
During an interview on 2/06/2023 ending at 2:02 PM, Personnel #8 was asked for re-education/training notes and sign in sheets after the event. Personnel #8 stated she did not have documentation.
