Based on medical record review and interview, the facility failed to ensure one Patient (Patient #15) was monitored in accordance with physician orders. This had the potential to affect all of the facility's 256 active patients.

Findings include:

The medical record for Patient #15 revealed a physician order for strict intake and output on 06/08/15 at 1:00 PM. The medical record did not contain evidence the facility monitored Patient #1's intake and output on 06/08/15 and 06/09/15.

The findings were shared with Staff N on 11/18/15 at 11:25 AM and confirmed.

Based on review of the facility's policy and staff interview, the facility failed to ensure staff followed the manufacturer's instructions for the cleaning and disinfection of glucometers. This had the potential to affect all of the facility's 256 active patients.

Findings include:

The facility's Cleaning and Disinfection of Non Critical Reusable Medical Equipment policy was reviewed. The policy stated the manufacturer's guidelines on cleaning and decontaminating a device are to be followed. The policy stated to wipe all surfaces of the glucometer with alcohol following each use. The policy stated to wipe all surfaces with a bleach wipe/solution should the unit become contaminated with blood/body fluids.

The facility's Waived Testing policy was reviewed. The policy stated all surfaces of the glucometer must be cleaned/disinfected with an alcohol wipe following each use. Wipe the unit with a bleach wipe/solution should the unit become contaminated with blood or body fluids.

The manufacturer's instructions for the cleaning of the glucometer were reviewed. The instructions stated to clean the meter with a cloth that has been dampened with a 10% bleach solution or disinfectant wipe.

On 11/19/15 at 8:16 AM, Staff M reported having called the glucometer's manufacturer who reported the facility was not cleaning the glucometers in accordance with the manufacturer's instructions by using alcohol wipes after patient use. Staff M reported the facility's policy was updated, staff were reeducated and all staff received an email regarding the change in cleaning the glucometers to require using bleach after patient use.
