Based on observation and staff interview, the facility failed to ensure that vertical openings were protected and maintained in accordance with NFPA 101-Life Safety Code.
Findings include:
On 01/09/19 at approximately 10:02AM, it was observed that the fire door of the stairwell connecting the Basement to the 1st and 2nd Floors in the Jennings Building did not have a fire rating label on it.
In addition, the required annual fire door inspection was not performed on this door.
An interview with Staff L (Director of Engineering) on 01/09/19 at approximately 10:03AM confirmed these findings.

Findings include:
Valentine Building:
On 01/07/19 at 11:10AM, it was observed that a concealed sprinkler head in the facility's Kitchen area was sticking out of the suspended ceiling in the Valentine Building. An interview with Staff L (Director of Engineering) on 01/07/19 at 11:12AM confirmed this finding.
Jennings Building:
On 01/08/19 at 11:11AM, it was observed that a pendant sprinkler head was painted in the 2nd Floor Supply Storage Room in the Jennings Building. An interview with Staff L on 01/08/19 at 11:12AM confirmed this finding.

Based on observation and staff interview, the facility failed to ensure that smoke barriers were constructed and maintained in good condition.
Findings include:
On 01/09/19 at 10:29AM, it was observed that an exposed iron beam was incorporated into the 1-hour smoke barrier by Patient Room #112 on the 1st Floor of the Jennings Building. An interview with staff L (Director of Engineering) on 01/09/19 at 10:30AM confirmed this finding.

On 01/07/19 at 10:59AM, it was observed that a duplex electrical receptacle on the Kitchen wall was broken. An interview with Staff L (Director of Engineering) on 01/07/19 at 11:00AM confirmed this finding.
