Based on a review of facility documents, medical records (MR) and interview with staff (EMP) it was determined that Meadows Psychiatric Center failed to follow their adopted Medical Staff Rules and Regulations by failing to complete a discharge summary within 12 days after discharge in four of eight medical records (MR2, MR3, MR4 and MR5).

Findings Include:

The Meadows Psychiatric Center and Universal Community Behavioral Health Medical and Professional Staff Organization Rules and Regulations, revised February 2018, was reviewed and revealed, "... E. The Discharge Summary is completed as soon as possible, but not later than 12 days after discharge. ... ."

1. Medical Record review was completed on May 2, 2018. It was noted that four of eight medical records (MR2, MR3, MR4 and MR5) failed to reveal documented evidence of a Physician Discharge Summary.
MR2 was discharged on April 16, 2018,
MR3 was discharged on April 16, 2018,
MR4 was discharged on April 4, 2018,
MR5 was discharged on April 16, 2018.

Interview with EMP3 on May 2, 2018, at approximately 1:00 PM confirmed the above findings.
