Based on observation and staff interview, the facility failed to ensure that supplies contained on the Station 2 Crash Cart were not outdated.

Findings Include:

Observations made on 01/14/2014 between 11:15 a.m. and 5:00 p.m. revealed the following outdated supplies on the Station 2 Crash Cart.

1) Pediatric Ambu Bag - date on packaging was 4/26/07. The bag and packaging were yellowed.
2) Endotrachael tubes - packaging was yellowed and peeling.
3) Intravenous fluids - two (2) 0.9% (percent) Sodium Chloride (NaCl) 1,000cc (cubic centimeter) bags expired November 1, 2012; 5% Dextrose 1,000cc bag expired July 1, 2011; two (2) 0.9% NaCl 250cc bags expired August 1, 2012.
4) Primary Intravenous Set - sticker on package was dated 11/22/05. Product was yellowed.
5) Secondary Intravenous Sets - sticker on three (3) packages was dated 11/22/05. Products were yellowed.
6) Easy cap CO2 (Carbon Dioxide) detector - expired 12/2013
7) LoPro Tracheal Tube - packaging yellowed and peeling.
8) Ambu Endotrachael Tube Holder - Date on package 1/30/11.
9) Suction Catheter Kit - Packaging yellowed and peeling.

On 1/14/14 at approximately 11:30 a.m. an interview was held with the Director of Nurses (DON) in her office. A list of all the outdated supplies found during observations was given to the DON. The DON acknowledged the supplies were outdated and stated they supplies were to be returned to Central Supply and the Station 2 Crash Cart was to be re-supplied immediately with appropriately dated supplies.
