Keyed locks, dead bolt locks and multi-latching devices create an impediment to egress from habitable spaces. Doors shall be operable with not more than one releasing operation. 7.2.1.5.4

The facility failed to ensure exit access was readily accessible at all times.

Observation determined:

1) The corridor door to the Pharmacy was a dutch door with the top and the bottom sections of the door latching into the door frame. The top half of dutch doors must latch into the bottom half of the door to assure only one operation is required to open the door.

2) The corridor doors to Office 123, Office 124, and the Director of Nursing were equipped with lever latches and dead-bolt thumb latches.

A functional test must be conducted on every required emergency lighting system at 30-day intervals for not less than 30 seconds. An annual test must be conducted on every required battery-powered emergency lighting system for not less than 1 1/2-hours. Equipment must be fully operational for the duration of the test. Written records of visual inspection and tests must be kept by the owner for inspection. 7.9.3

The facility failed to assure the emergency lighting battery pack would pass a 90 minute continuous test.
Observation determined the battery pack emergency lighting adjacent to the emergency generator in the penthouse failed the 90 minute continuous test.
