Based on clinical record review and interview, it was determined the Facility failed to disclose the risks and benefits of transfer specific to the condition of four (#1, #7, #8 and #21) of four (#1, #7, #8 and #21) transferred patients. Failure to disclose the risks and benefits specific to the condition did not allow Patients #1, #7, #8 and #21 to make an informed decision regarding the need for the transfer. The failed practice was likely to affect Patients #1, #7, #8 and #21. Findings follow:
A. Review of the clinical records of Patients #1, #7, #8 and #21 revealed no documentation of the risks and benefits specific to the condition for transfer.
B. The Director of Nursing was interviewed at 1611 on 12/16/14 and the findings of no documentation of the risks and benefits specific to the condition of the patient upon transfer for Patients #1, #7, #8 and #21 was verified by the Director of Nursing at that time.
