Based on observation during the morning of 9/21/12, the 8'-0" wide egress corridor at the north end of the second floor had a cross-corridor glass wall (with one door in it) installed between the patient sleeping rooms and the stairway exit door.

Based on observation and interview with the Plant Operations employee during the afternoon of 9/20/12, the fire alarm system required maintenance. The "trouble" signal on the main fire alarm control panel was illuminated to indicate a malfunction in the system.
