Based on interview, review of employee files, and review of the facility's policy manual, it was determined the facility failed to comply with their policy related to Background checks for one (1) employee (Employee #6).
The findings include:
Review of the facility's policy for "Background Investigations", dated 08/99 and revised 09/17, revealed background investigations were completed on candidates for employment and individuals who provided clinical services under contract.
Review of Employee #6's file revealed he was hired on 10/01/99. Further review of the employee file revealed there was no documented evidence a background investigation had been completed.
Interview with the Vice President of Human Resources, on 11/21/17 at approximately 8:47 AM, revealed she had no documented evidence a background investigation had been completed for Employee #6.
