Based on surveyor observations, it was determined that the facility has failed to provide complete sprinkler coverage for all portions of the building as required by NFPA 101 Section 19.3.5.3.
Findings are as follows:
Surveyor observations on the morning of November 13, 2012, in the presence of the Director of Support Services, revealed that the 2nd floor dialysis supply closet, adjacent to the dialysis unit, and the 3rd floor linen closet on the endoscopy unit were not provided with fire sprinkler protection.
The facility Director of Support Services confirmed the observations.

Based on surveyor observation it was determined that the hospital failed to ensure that the electrical wiring and equipment was in compliance with applicable requirements of NFPA 70, National Electrical Code.
Findings are as follows:
1. Surveyor observations on the morning of November 14, 2012, in the presence of support services staff of the Westerly Hospital Outpatient Rehabilitation Services, 81 Beach Street, Westerly, RI revealed that the electrical closet, adjacent to the exit door - gym side, contained two large boxes of paper and 11 large blue binders of computer paper on the floor.
Storage of combustible materials is not allowed in electrical rooms.
2. Surveyor observations on the morning of November 14, 2012, in the presence of the exercise physiologist of the Westerly Hospital Outpatient Services, 45 Wells Street, Westerly, revealed that the electrical closet on the first floor, contained three large piles of paper.
