NFPA 96: Based on visual observation and record review the facility failed to assure that deep fat fryers and open flame appliances shall have 16" clearance between the fryer and the open flame appliance or a baffle/metal shield 8" installed between the fryer and the appliance. The deficient practice had the potential to affect 24 of 24 residents.

Findings:

During the visual observation, between the hours of 7:30am to 12:00pm revealed that a new stove was installed and the baffle/metal shield between the open flame stove and the deep fat fryer was not installed. The nozzle covering the deep fat fryer does not appear to be pointed to cover the entire fryer.


Interview with maintenance supervisor revealed the facility was not aware the appliances in the kitchen were not covered on the hoop suppression system and the baffle/metal shield between the fryer and the stove was not replaced.

Nfpa 1: 10.18.5.1 Combustible material shall not be stored in mechanical rooms, boiler rooms, or electrical equipment rooms.

Based on visual observation the facility failed to assure that was not any combustible storage in the mechanical room. The deficient practice had the potential to affect 24 of 24 residents.


Findings:

During the facility tour, between the hours of 7:30am to 12:00pm revealed that combustible storage existed in the mechanical room.

Interview with maintenance supervisor revealed the facility was not aware the mechanical room has combustible storage.

Gas and Vacuum Piped Systems - Inspection and

Nfpa 99: All Central supply systems must be fully inspected yearly , including all of the major components and maintained free of deficiencies. Based on visual observation and record review the facility failed to repair the deficiencies of the inspection conducted 9/30/2021. The deficiency has the potential to affect 24 of 24 residents.

Findings:

During the record review , between the hours of 7:30am to 12:00pm revealed that the med gas system was inspected on 9/30/2021 but the deficiencies were not corrected.

Interview with maintenance supervisor revealed the facility was not aware that the med gas deficiencies were not corrected.
