Findings include:
On survey of June 30, 2010, hospital staff verified that there is no designated organization structure for this hospital. See Tag C-241.
The cumulative effect of this failure to have a designated governing body resulted in the facility's inability to ensure the conduct of hospital operations.

Based on two of two staff interviews by surveyor #13469, the hospital failed to have a governing body at Saint Mary's Hospital, Superior, WI hospital/campus.
Findings include:
Per interview, with Hospital Administrator (A) on 6/30/10 at 9:30 AM, (A) said that this hospital, Saint Mary's Hospital, Superior, WI, is owned and operated by the Saint Mary's Hospital, Duluth, MN. Per (A), the Duluth, MN hospital has the governing body that oversees this, St. Mary's Hospital, Superior, WI, hospital/campus.
Hospital Administrator (A) said the Superior, WI hospital/campus does not have an established governing body, and per (A), there are no representatives from the Superior hospital/campus that attend the Duluth MN governing body meetings that are held monthly.
Per interview, with Vice President of the Duluth, MN hospital (B) on 6/30/10 at 11:00 AM, (B) said there is a governing board that meets and expects a representative from the Superior, WI hospital to provide information to the Duluth, MN governing body, but confirmed the Superior, WI hospital does not have their own governing body.
