Based on observation and interview, the facility failed to ensure that corridors were maintained free from obstructions to full instant use in the case of fire or other emergency. This deficient practice affected one (1) of ten (10) smoke compartments, staff, and twelve (12) patients. The facility has the capacity for twenty-five (25) beds with a census of twelve (12) on the day of the survey.

The findings include:

During the Life Safety Code tour on 12/06/16, at 11:35 AM with the Director of Maintenance (DOM), rows of chairs were observed in the corridor at the first floor back hallway. Corridors must be kept clear and unobstructed for exiting purposes in case of fire or other emergency.

An interview with the DOM on 12/06/16, at 11:35 AM revealed he was not aware these corridors must be kept clear and unobstructed.

The findings were revealed to the Administrator upon exit.

Corridor - Doors

Based on observation and interview, the facility failed to ensure that corridor doors were maintained free from obstructions to full instant use in the case of fire or other emergency. This deficient practice affected one (1) of ten (10) smoke compartments, staff, and twelve (12) patients. The facility has the capacity for twenty-five (25) beds with a census of twelve (12) on the day of the survey.

The findings include:

During the Life Safety Code tour on 12/06/16, at 11:20 AM with the Director of Maintenance (DOM), the doors to patient rooms 210 and 211 located on the second floor were observed not to be able to close and latch as required due to isolation door racks installed on the doors. These doors must be able to close and latch to help prevent fire and smoke from spreading to other parts of the building in an emergency.

An interview with the DOM on 12/06/16, at 11:20 AM revealed he was aware the doors must close and latch; however, he was not aware the racks prevented the doors from closing and latching as required.

The findings were revealed to the Administrator upon exit.
