Based on observation and interview, the hospital does not have all exits marked with signage.
Findings include:
Observation on 07/14/14 revealed that in the three-story stairway, off the Hodge building, the exit door within the stairway leading to the outside was not marked with a sign, nor was it obvious that it was the exit.
This finding was verified with Director of Quality Management Staff #2 on 07/14/14.

Based on observation and interview, not all exit stairways are enclosed.
Findings include:
Observation on 07/09/14 revealed the single-story stairway, located near the boiler room, which discharges to the outside and is marked with an exit sign, did not have a door between the stairway and the corridor outside the boiler room. During interview, Director of Facilities Management Staff #3 stated that the door had been removed some time ago, and the stairwell is not needed as part of the required egress.
This finding was verified with Director of Quality Management Staff #2 on 07/09/14.

Based on observation and interview, the facility does not maintain all stairways.
Observation during tour on 07/14/14 revealed:
- The seven-story stairway in S-Building did not have stair identification signs indicating the terminus of the top and bottom of the stair enclosure, the identification of the stair enclosure, or the story of and direction to the exit discharge.
- The landing between the second and third floors of the three-story stairway off the Hodge building had approximately 30 cigarette butts on the floor of the landing. The cover to the radiator in the landing had been removed and presented a potential tripping hazard on the floor of the landing.
These findings were verified with Director of Quality Management Staff #2 on 07/14/14.

Based on observation and interview, the facility does not maintain all means of egress free of all obstructions and impediments.
Observation during tour on 07/10/14 revealed:
- The exit corridor outside of the ultrasound department, which terminates with an exit door to the outside, contained furniture on both sides of the corridor. There was a two-person couch in the corridor adjacent to the door to ultrasound, and on the opposite wall there was a two-person couch and two chairs. One of the chairs projected into the pathway of the exit door.
- The exit door that discharges to the outside near the laundry and the cooling tower had a 20-foot steel ladder near it, which projected about two feet into the path of egress. Continuing along that path of egress, it was noted there was a 20-foot extension cord lying across the sidewalk. One end of the cord was plugged into an outlet, and the other end was not connected to anything.
- By the stairwell located outside of the boiler room, two eight-inch water lines, used for cooling, were noted to be outside three feet from the exit door. The water lines were approximately three inches off the ground, perpendicular to the path of egress. Director of Facilities Management Staff #3 stated that the cooling lines were put in place when the original lines, which were underground, had broken. Staff #3 stated that the water cooling lines are intended to be temporary.
These findings were verified with Director of Quality Management Staff #2 on 07/10/14.
