In the last six months, the facility's emergency department saw 33,988 patients and 508 patients were transferred to other facilities.

Based on record review and interview, the facility failed to ensure individuals transferred to another hospital had a physician signed certification with a summary of the risks and benefits of the transfer. This affected one (Patient #1) of seven medical records reviewed (Patient #1). In the last six months, the facility's emergency department saw 33,988 patients and 508 patients were transferred to other facilities.

Findings include:

Review of the medical record for Patient #1 revealed an arrival date of 11/27/24 at 12:51 AM to the emergency department. The medical record contained documentation the patient had preterm premature rupture of membranes with unknown onset of labor. The patient requested to be transferred to a different acute care hospital where her physician was. The patient was transferred to the requested hospital on 11/27/24 at 3:54 AM.

Review of the "Patient Transfer Form" for Patient #1 revealed the physician checked the reason for transfer as patient request and the risks and benefits of being transferred have been explained to the patient. The physician signed and dated the form. However the Risks and Benefits section did not have any of the boxes checked and nothing was written in this section.

During an interview on 01/08/25 at 4:15 PM, Staff K verified the above findings.
